Is it possible to fail mixing items?

well my formula was right 8 sapphires, 2 oredoes, and 2 bellums. Yet i failed, i was kinda shock like wtf... lol That was my reaction. I posted in recovery center, hopefully gm will do something aabout it.

I have a doubt about mixing, and I couldn't manage to find the info into the guides. Theres a way to "de-mix" an item, to remove the mix? Because a clan buddy gave a Scythe, but it's for Pike and I'm a mech, I want to change the spec with Nied.
