How does this skill work? Sometimes it does massive damage and sometimes it does no damage. How close / far do you have to be?

The radios that deals most of the damage is exactly at the point of impact of the bolt - try to aim your enemies on that area or just a little below.

 

The reason why there are areas with low damage, is because, each bolt has an radius too. That radius is small and, they don't strike at the same position, they move a little bit to the side. So, dependind on the position of the monster, some may get not hit.
