Aging Chances

Does anybody know the aging chances for each age level?

Knowing the de-age & breaking %'s would be helpful too.

I know +1 to +6 is 100% but beyond that?

Is there some kind of table/chart with the %'s on?

 

I've tried looking but I can't find anything on the specific numbers.
